This is a tough case where the patient has an unusual eye with a very small anterior segment but a long axial length with a large vitreous cavity. The patient needs to have cataract surgery for visual rehabilitation and also to facilitate the view of the retina which shows an extensive retinal detachment. The anterior chamber depth is 1.99 mm with a small white-to-white measurement of 10.8 mm. The lens is microspherophakic with a thickness of 6.14 mm from anterior to posterior. What would be your approach to this cataract surgery? And why does the capsulorhexis go from perfectly round at the beginning of the case to oval shaped at the end?
