Procedure to Fill Application Form
Before filling this form, Kindly arrange the Scan Copy of following documents ready with you:-
Scan copy of old certificate
Structural Layout of Building
Address Proof of the Establishment
Passport Size Photograph of Person In-charge
Certificate from Medical Council of India/State Medical Council
Self Attested Documents of Relevant Qualification for Doctor & Paramedical Staff.
NOC Certificate from Pollution Control Board
NOC Certificate for Disposal of Medical Waste
Photo for Proof of Fire Fighting System Installed In the Premises of Establishment
Notarized Affidavit on Stamp paper of Rs. 10.
Note:-
All files should be uploaded in jpg/pdf format and maximum file size allowed up to (2MB)
Fields marked with * are mandatory.
Fill all the required fields & attached the relevant documents then click on “Save” button.
A preview page will appear, now check all the details carefully filled by you.
If every details is correct & no change is required then click on “Submit” button
Note down your Application Reference number for future use.
Download Formats with content of Affidavit and get it printed on the Stamp Paper of Rs. 10/-
Now, re login & upload the notarized affidavit as required & click on “Final Submit” button to submit the application finally.
You will be redirected to the new window, Now Click on the “Print Your Submitted Application” tab to get the printout.
You can check the current status of your application by clicking on the “View Submitted Application Status”
Terms & Conditions
It is the right of nodal officer to accept or reject the application form by mentioning the “cause of rejection.”
Nodal officer will appoint the inspection committee & schedule the date of Inspection, applicants need to be physically present in the establishment on the date of inspection.
Applicants should help the inspection committee & will show the originals of the document attached during submission of application form.
Applicant should also handover the original Affidavit to the inspection committee with all copy of all attached documents.
Inspection committee should upload its inspection report in the portal.
Based on the inspection report CMO is free to approve or reject the application
If application will be approved then Computer Generated Registration Certificate will be made available on the dashboard of applicants.
Applicants can also collect their computer generate registration certificate from the office of Chief Medical Officer.
Validity of Certificate will be 30th April of the next year from the date of Issuance.
Applicant can apply for renewal of registration certificate through their login but before the 30th April of next financial year.
