क्या rectal cancer में हर patient को stoma की जरूरत पड़ती है?
Rectal Cancer और Stoma
सबसे पहले समझिए, Rectal cancer मतलब बड़ी आंत (colon) के सबसे आख़िरी हिस्से, rectum में tumor होना।
Rectum का ये हिस्सा करीब 15 सेंटीमीटर लंबा रहता है और इसे हम तीन हिस्सों में बांटते हैं
Upper rectum (ऊपरी 5 सेंटीमीटर)
Middle rectum
Lower rectum (आख़िरी हिस्सा, anus के पास)
कई बार patients मेरे पास आते हैं, कह चुके होते हैं कि "doctor ने stoma की बात की है, अब पेट पर bag लगेगा क्या?" असल में, stoma मतलब stool passage temporarily या permanently पेट की skin पर लाना।

Stoma की जरूरत क्यों पड़ती है?
Rectal cancer में stoma ज़रूरी है या नहीं, ये tumor के location, spread और बाकी treatment पर depend करता है।
अगर cancer upper rectum में है, तो आमतौर पर stoma rarely ही चाहिए।
Middle या lower rectum के tumors में, खासकर अगर tumor anus sphincter तक पहुंच चुका है,
तो कई बार stoma की ज़रूरत पड़ सकती है।

Stoma के दो टाइप्स:
Temporary stoma (आमतौर पर ileostomy):
जब rectum का tumor निकालते हैं, फिर बड़ी आंत को नीचे जोड़ते हैं,
तो healing के लिए stool का passage कुछ हफ्तों (6–8 weeks) के लिए पेट पर निकालते हैं।
बाद में सब ठीक रहा, तो इसे बंद कर दिया जाता है।

Permanent stoma:
अगर cancer इतना नीचे है कि anus sphincter भी involve हो गया है,
यानी control करने वाली muscle भी tumor में है
तो पूरा anus निकालना पड़ सकता है। तब stool passage permanently पेट पर लाना पड़ता है।
लेकिन आजकल permanent stoma का rate बहुत कम है, 5% से भी कम cases में।

कब stoma ज़रूरी है, Doctor का फैसला
हर patient को stoma लगेगा, ऐसा ज़रूरी नहीं है।
यह सिर्फ tumor के size, location, और pre-surgery treatment (radiation, chemotherapy) पर depend करता है।
Lower rectum या radiation/chemotherapy के बाद वाले cases में
surgeon safety के लिए diversion stoma (temporary) बना सकते हैं।
कभी-कभी जब healing या leakage risk हो, तब भी temporary stoma लगता है।
Upper rectum में cancer हो तो अक्सर stoma की जरूरत नहीं पड़ती है।
यह फैसला हमेशा आपके surgeon को करना चाहिए, क्योंकि surgery के दौरान ground reality वो ही समझ सकता है।
Patient का best interest और safety हमेशा surgeon की priority होती है।
