नमस्ते दोस्तों! मैं Dr. Anil Singhal, Senior Orthopedic & Robotic Joint Replacement Surgeon, OrthoCare Hospital, Bulandshahr से। आज हम डिस्कस करेंगे एक Spinal Injury का केस। इस केस में एक 35 साल का male patient घर पर काम करते हुए छत से गिर गया। काफी ऊँचाई से गिरने के कारण पेशेंट को कमर के ऊपरी हिस्से में severe pain और movement की कमी थी। हमने पेशेंट का X-ray और MRI कराया। इसमें पता चला कि D3 से D6 vertebrae में fracture हुआ है और spinal cord के पास हड्डियां पहुंच गई थीं। अगर सही समय पर fixation नहीं किया जाता, तो पेशेंट के हाथ-पैर की movement प्रभावित हो सकती थी।

Case Overview:
एक 35 वर्षीय male patient मेरे hospital में आया। वह घर पर काम कर रहा था और अचानक छत से गिर गया। गिरने की height काफी थी, और पांच दिन पहले हुई इस घटना के बाद वह severe pain में था।

Injury Details:
Patient को upper back/कमर के ऊपरी हिस्से में गंभीर दर्द था और movement करना मुश्किल था। X-ray और MRI से पता चला कि D3 और D6 vertebrae के बीच की हड्डियां crush हो गई थीं। Spinal cord के पास हड्डियां touch कर रही थीं, जिससे high neurological risk था।

MRI & Diagnosis:
MRI से स्पष्ट हुआ कि D1–D6 vertebral region में collapse और fracture हुआ था। हालांकि, patient के हाथ और पैर अभी भी move कर रहे थे और neurological deficit नहीं था।

Neurological Status:
Spinal injury में यह जानना बहुत important है कि neurological deficit है या नहीं। इस केस में, patient के हाथ और पैर normal movement कर रहे थे, इसलिए neurological status अभी safe था।

Imaging & Assessment:
Spinal injury का assessment X-ray, MRI और CT scan के द्वारा किया जाता है। इससे determine किया जाता है कि injury stable है या unstable, और भविष्य में neurological problems की संभावना कितनी है।

Neurological Consideration:
अगर neurological risk हो सकता है, भले injury stable दिखे, तो fixation की recommendation दी जाती है। इस केस में, small movement भी difficult थी और potential leg weakness का risk था।

Surgery Indication:
Patient को fixation के लिए preference दी गई ताकि हड्डियां stable रहें और spinal cord पर load न पड़े।

Surgical Procedure:
Operation में pedicle screws और rods का इस्तेमाल किया गया।
Upper और lower vertebrae को stabilize किया गया।
Fracture वाले हिस्से पर कोई load नहीं पड़ा।

Recovery Process:
धीरे-धीरे biology अपना काम करती है और हड्डियां heal होती हैं।
Patient normal walking और daily activities resume कर सकता है।
Proper physiotherapy और care के साथ recovery ensure होती है।
