So we are back to our regularly scheduled program with nurse vids 😁

Here are some tips on how to appropriately document as s hospice nurse 👩‍⚕️
This is just some way to document as a hospice nurse

We ALWAYS do a head to toe assessment (as you would in any healthcare setting) and chart our findings based off of the had to toe as well as patient/caregiver report.

Remember, as a hospice nurse you will be seeing your patients once a week minimum (Medicare only requires an RN once every two weeks but most hospices will do nursing visits at LEAST once a week whether that be an LPN or RN)
So you will need to acquire a picture of what your patient’s week or few days have been since you’ve seen them last and “paint” a picture of your patients “story” through your note.

Now there may be different terms you will utilize in your practice and you may not use these terms at EVERY visit (or you might) depending on your assessment of the patient.

Please always follow your agency’s rules when it comes to charting.

And chart like it may be read out loud in a deposition one day
