1. Transferrin is difficult and expensive to measure directly
2. Total iron binding capacity (TIBC) is a surrogate for transferrin
3. If iron levels are low, your body makes more transferrin and TIBC will go up!
4. NOTE: Transferrin is a negative acute phase reactant and therefore will go down in states of chronic inflammation or cirrhosis. Therefore, TIBC may be low in these states, even if the patient has iron deficiency anemia!
