Mycobacterium tuberculosis bacilli will be engulfed by pulmonary macrophages and then will be processed inside macrophages into peptides (primary protein). Peptides derived from mycobacterium tuberculosis bacilli will bind major histocompatibility class II (MHC II) at the rough endoplasmic reticulum. Peptides in association with MHC II get expressed on the surface of macrophages. Th1 cells are blind cell and can not see tertiary proteins, accordingly they can see primary peptides following processing of tertiary protein into peptides. Th1 cells through their TCR interact with these peptides and get activated. Activated Th1 cells produce interferon gamma that in turn recruit more macrophages to the area. Recruited macrophages wall off the remaining of bacilli. Activated macrophages will express tumor necrosis factor alpha that has the capacity to destruct bacilli and nearby healthy cells resulting in circular regions of necrotic tissues which form a lesion with a caseous consistency (cheesy like). Some of macrophages coalesce to form giant cells that have the capacity to deal with interactable bacilli. Also some of macrophages modified into epithelioid cells that have the capacity to wall off persistent bacilli.
