A RadioShack Remote Controlled Outlet model 61-2667. This unit likely dates back to at least the early 2000s, probably even older than that. By some miracle, both pieces are still together after all these years.

The box plugs into an outlet and has an outlet on it which is controlled by the remote. It is an RF remote, and while it can transmit through a very small amount of obstructions, the range is very limited. It's good enough to work within the same room, but not much beyond that.
