The skull base is literally the floor of the vault of the brain. The bony vault that protects the brain. Incidentally the other side of the wall with the nasal cavity and the orbits. So skull base surgery means going into the floor of the brain per se and dealing with issues there. When that was done externally it was not referred to by anything other than the name skull base. The nomenclature endoscopic skull base came with the advent of endoscopes in identifying the sane areas with minimal access through the nasal cavity to a wide area which covers the orbit as well as the nasal roof which is the skull base. So endoscopic skull base surgery is the use of an endoscope to go into the roof of the nose or the orbit to deal with lesions which may be in the base of the skull.
