The uterus is made up of 2 layers, the outside layer is the muscular layer and the inside layer is the inner lining, which is called as the endometrium. It really depends on the ultrasound. Ultrasound is a really important tool, which differentiates which one of these layer is enlarged. It could be that the inside layer of the uterus, the endometrium is abnormally enlarged or increased. So that is abnormal. So after we finish our periods, the inside lining is about 4mm. Every day it goes, hoping that if there is a pregnancy, it can act as a cushion to avoid the pregnancy. But it never crosses around 14 or 15 mm. More than that is it is abnormal at many stage. If the muscular wall of the uterus is abnormal it could be because of a variety of reasons, it could be because of fibroids or adenomyosis or infection. But not all enlarged uterus is abnormal. A women could be born with a slightly bulky uterus, she could develop a bulky uterus as reaction or a response to babies, a response to infections or a reaction to periods not all bulky uterus is necessarily need treatment. Symptomatic bulky uterus where the patient is suffering, will need conservative or surgical management as may be the case.
