Missed miscarriage 12 week ultrasound

I thought I was 11w6d and at my OB appointment was told there is no heartbeat and baby stopped growing at 9w1d on an abdominal ultrasound. My first appointment was at 8w3d and we saw a heartbeat 164 bpm and everything looked perfect. Doctor couldn't tell me why this happened. I have not had any symptoms of a loss when this video was recorded on 7/31/2015

UPDATE: I gave birth to our Rainbow Baby January 17, 2017.
