The serum transferrin receptor (sTfR) test measures the concentration of receptor proteins in the blood, reflecting cellular iron needs, particularly to distinguish iron deficiency anemia from anemia of chronic disease. It rises when iron is low and is unaffected by inflammation, unlike ferritin. It is often paired with, but distinct from, transferrin saturation (TSAT), which measures iron binding capacity.
Key Details About Serum Transferrin Receptor (sTfR) Testing
Purpose: The sTfR test indicates how aggressively body cells are trying to take up iron.
Significance: Increased sTfR levels indicate that the body is in an iron-deficient state and attempting to increase absorption.
Advantage in Diagnosis: Unlike ferritin, which can be falsely elevated by inflammation, sTfR remains high in iron deficiency, making it effective for identifying anemia in patients with chronic infections or inflammatory diseases.

A serum transferrin saturation (TSAT) test measures the percentage of the iron-transport protein transferrin that is bound to iron, indicating how much iron is available for bodily processes. It is a key indicator of iron deficiency (low TSAT) or overload conditions like hemochromatosis (high TSAT). Generally, 20%-50% is considered normal.

Utility of sTFR/Ferritin index to differentiate iron deficiency anaemia and anaemia of chronic diseaseIron deficiency is a common condition that is usually diagnosed using conventional laboratory tests of iron status, such as serum ferritin and transferrin saturation. However, both ferritin and transferrin proteins are markedly influenced by inflammation, behaving as acute-phase reactants and making it difficult to differentiate between iron-deficiency anemia (IDA) and anemia of chronic disease (ACD).Level of sTfR is markedly elevated in Iron deficiency anaemia but remains normal in anemia due to chronic inflammation without iron deficiency. Thus levels of sTfR may be of considerable help in differentiating between Iron deficiency anaemia and Anemia of chronic disease. In addition, use of the formula sTfR/log ferritin ratio may increase the efficacy of sTfR in identifying Iron deficiency anaemia and Anaemia of chronic disease.
