High-intensity shockwave therapy, commonly used for kidney stones, has shown promise for treating Peyronie’s disease. In this educational video on UroChannel, Dr. med. Dr. phil. Stefan Buntrock, a board-certified urologist and sexologist from Göttingen, explores the pros and cons of this treatment. Peyronie’s disease causes the penis to bend during erections due to hard, calcified plaques that form in the tissue. This condition affects about one in ten men and can be severely debilitating.
Shockwave therapy, initially developed for kidney stones in the 1980s, uses high-energy waves to break down these calcified plaques. While low-intensity shockwaves for erectile dysfunction don't have the necessary energy, high-intensity shockwaves can effectively target and crush the plaques. Dr. Buntrock shares a case where a patient experienced significant improvement in penile curvature and erectile function after undergoing Extracorporeal Shockwave Lithotripsy (ESWL).
A recent study published in the World Journal of Urology supports these findings. It followed 110 patients treated with ESWL from 1996 to 2020. Results showed that 50% of the patients experienced a significant reduction in curvature, especially those with an angle between 30-60°. Additionally, erectile function improved notably, with median IIEF scores rising from 14 to 21 points, approaching normal function. However, 16% of patients reported a deterioration in their condition.
Dr. Buntrock discusses the challenges and limitations of ESWL, including the difficulty in targeting plaques, the need for anesthesia, and the bruising that follows treatment. He emphasizes that while ESWL shows promise, it is only suitable for select patients and should be administered under professional guidance.
