In this case we have an 83 year old man with recurrent squamous cell carcinoma (SCC) of the scalp. Resection of the tumour resulted in loss of part of the underlying periosteum and the exposure of bare bone. There was not enough tissue laxity to allow repair using a skin flap and so the defect was repaired with a skin graft. As expected, the graft over the bare bone failed.
