The ECHO-2 Grenade Mod is a unique white rarity grenade mod in reference to Claptrap's speech at the beginning of the game. Huge thanks to KingPan94 over on Twitter for showing me this thing and pointing it out to me. I managed to get one to drop from both Graveward and Katagawa Ball and KingPan94's dropped from Captain Traunt on the planet of Athenas so this thing may be a random drop from the bosses of Borderlands 3, it seems.
