नमस्कार दोस्तों, मैं Dr Sumit Shah, Cancer Surgeon यानी Surgical Oncologist, Prolife Cancer Center & Research Institute, गुलटेकड़ी मार्केट, पुणे से।
आज हम एक बहुत ही ज़रूरी विषय पर बात करने वाले हैं, Rectal Cancer की Surgery के बाद मिलने वाली Final Histopathology Report को

कैसे पढ़ें और क्या-क्या बातें समझना ज़रूरी होता है?
Report का पहला हिस्सा: Specimen
सबसे पहले report में Specimen लिखा होता है।
इसमें mention होता है:
Low Anterior Resection
और Distal Donut
मतलब आपने rectum का जो हिस्सा निकाला है,
वो और जो distal resection ring होता है, वो दोनों lab में भेजे गए हैं।
यह distal donut इसलिए भेजा जाता है ताकि ये confirm किया जा सके
कि जो rectum का निचला हिस्सा था, वहां भी कोई microscopic tumor residue तो नहीं है।
Clinical History & Biopsy Report
इसके बाद आता है patient की clinical history और pre-surgery की biopsy का reference।
ये लिखा जाता है ताकि pathologist को disease की background समझ में आ सके।
Gross Examination: आँख से दिखने वाला पार्ट
फिर आता है Gross Examination यानी बिना microscope के जो दिख रहा है।
इसमें ये सब देखा जाता है:
Total specimen की length कितनी है?
Tumor की exact location और size क्या है?
क्या tumor ऊपर की तरफ peritoneal surface तक पहुंचा है या नहीं?

Tumor में कोई perforation है क्या?
इसके साथ-साथ pathologist ये भी note करते हैं कि:
Proximal और Distal margins tumor से कितनी दूर हैं
और सबसे ज़रूरी Completeness of TME यानी
Total Mesorectal Excision complete हुआ या नहीं
अगर mesorectal envelope कहीं से damage हो गया हो, तो risk बढ़ सकता है कि
cancer के microscopic cells पीछे छूट जाएं।
Microscopic Findings: असली सच्चाई
इसके बाद report का सबसे important पार्ट आता है Microscopic Examination यानी histology।
इसमें ये देखा जाता है:
Tumor grade –
Well differentiated, Moderately differentiated या Poorly differentiated
Tumor size –
Exact millimeters में ट्यूमर कितना बड़ा है
Depth of invasion

Tumor सिर्फ mucosa में है या submucosa, muscularis तक पहुंचा है?
Margins clear हैं या नहीं
Proximal
Distal
और सबसे critical Circumferential Resection Margin (CRM)
Lymph node yield
कितने lymph nodes निकाले गए

और उनमें से कितने में tumor cells पाए गए?
एक Example रिपोर्ट को समझिए
मान लीजिए एक patient की report में लिखा है:
Well differentiated adenocarcinoma
Tumor सिर्फ submucosa तक गया है
कोई भी lymph node में metastasis नहीं है
और CRM negative है
तो इसका मतलब ये है कि disease एकदम early stage पर पकड़ा गया है।
इस stage को medically कहते हैं: T1 N0

T1 N0 का मतलब?
T1 – Tumor सिर्फ intestinal wall की पहली layer तक limited है
N0 – कोई भी lymph node में spread नहीं है
ऐसे patients में सिर्फ surgery ही काफ़ी रहती है।
ना chemotherapy की ज़रूरत होती है, ना radiation की।

कब लगता है आगे का इलाज?
अब सोचिए अगर:
Tumor deeper तक गया हो — जैसे T3 या T4 stage
Lymph nodes में spread हो — N1 या N2
या CRM positive आ जाए — यानी microscopic cells edge पे छूट गए हों
तो ऐसे patients को surgery के बाद adjuvant treatment देना पड़ता है:
Chemotherapy
Radiation therapy
या दोनों
