In this video I share more information. This time about extremely unethical and criminal actions by APO Staff in collusion with Case Management, and Contractors paid by the Connecticut Mental Health Dept., were all responsible for malpractice, abuse and neglect of a patient. At the time I did not realize how dangerous this medication was, they clearly needed me to go for an appointment on 10-15-2015 when I called and indicated I was having problems, and they should have had me come in immediately on 11-04-2015 when I indicated severe symptomology, instead they "created" a falsified office visit record for 11-05-2015 indicating fraudulently symptoms that have zero to do with a Hyperreaction to Triumeq, made it look like I was doing just fine with petty complaints. Being a victim of this falsification I can tell you that the Treatment SOP I experienced and still am is horrific.
10-15-2015 Missing Nurses Visit by Phone. Simply omitted.
11-04-2015, Nurses Visit by Phone did not list my symptoms, made it appear that I simply "Demanded" to stop (with no good reason? more delusion?) and falsified what I relayed, instead the record claims I indicated it made Peripheral Neuropathy "100% worse".
11-05-2015, Fraudulent, office visit record was created for an appointment that did not occur. They essentially attempted to sweep under the rug my Hyperreaction to Triumeq and that no contact had occurred on 10-15-2015, well it did and the prescription is hard evidence of the Nurses Visit by Phone. I provided this evidence in written complaint provided to multiple parties here in Missouri.
On 12-01-2015, a standard office visit, I had multiple complaints. Some were noted, others excluded. Dr. Adams who saw me did not sign the record, the NP signed off 10 days later. I had specifically requested this nurse NP not be involved in my medical care after a May 21st, 2014 office visit, she did not see me again. Dr. Adams exhibited bizarre behavior during this 12-01-2015 appointment. He was literally staring at his computer with a flushed face. He also performed an abdominal exam in a rough manner. At the end of the appointment he literally stood in front of me and got in my face as if to stare me down.
