In a young adult female, ultrasound imaging reveals a 1.5 cm cystic mass located above the thyroid isthmus, consistent with a possible thyroglossal duct cyst. The lesion appears wider than tall, with a soft tissue nodule inside, showing mild internal vascularity on color Doppler. No calcifications are present, and the thyroid gland is normal. Differential diagnoses include a thyroid nodule with cystic degeneration, dermoid cyst, lymph node with cystic changes, or ectopic thyroid tissue. The absence of calcifications and the midline location support a thyroglossal duct cyst, though further evaluation with fine-needle aspiration (FNA) may be needed for confirmation.
