American Airlines Flight 191, a DC-10 was a flight between Chicago and Los Angeles on May 25, 1979 with 271 people on board.

The Crash:
At 2:50pm, the flight pushed back from the gate at O'Hare International Airport and was cleared to taxi to runway 32R. Maintenance did not notice anything unusual during pushback or taxi. After the plane was cleared for take-off, it started its roll down the runway. Shortly after the pilots lifted the aircraft off, the engine #1 pylon bounced up and down and then separated from the jet. A smoke trail was formed after the engine ripped off from the wing. The separation caused damage to the left wing and some hydraulic systems, making the plane uncontrollably turn to the left. ATC tried to make contact, but failed to talk to the pilots. The plane then started to descend when it reached 350 feet and then crashed into an old aircraft hangar killing all 271 passengers and crew on board with another two people on the ground getting fatal burns. The engine detachment was due to bad maintenance.
