Ovarian cancer primarily spreads to the peritoneum. Ovarian cancer originates on the surface of the ovaries, allowing it direct access to the abdominal cavity from the outset. Within this abdominal cavity, there are numerous organs extending from the diaphragm to the pelvis, all of which are enveloped by a delicate membrane known as the peritoneum.

Consequently, when ovarian cancer cells spread by shedding into the abdominal cavity, they essentially land on this peritoneal membrane. This peritoneal spread is why it's the most frequently observed mode of ovarian cancer metastasis.
