A new Montana law looks to overhaul how pharmacy benefit managers operate, aiming to boost drug price transparency and protect independent pharmacies struggling under current reimbursement practices.

Originally the law was House Bill 740. It requires PBMs to reimburse pharmacies at transparent, standardized rates that reflect the actual cost of medications.

The new law eliminates “spread pricing,” a practice in which PBMs charge health plans more than they reimburse pharmacies and pocket the difference.

Now, employers and health plans are billed the same amount PBMs pay pharmacies, which allows Montanans to see true prescription costs.
