Conventionally, blocks in arteries of the heart or coronary arteries are diagnosed or visualized by a coronary angiogram, where dye is injected into these arteries by means of a catheter. Subsequently angioplasty and stenting are also performed with help of this dye for visualizing the balloon, stent etc.

Intra-Vascular Ultra-Sound is a technology by which ultra-sound waves are emitted through a fine catheter, which is passed inside the coronary artery (artery supplying blood to the heart). The sound generated helps in obtaining clear images from inside the artery.
Optical Coherence Tomography is a technique by which near-infra red light, instead of sound is used to visualize the inside of the arteries.
The images obtained by either technology helps to see the details off the block as presence and percentage of fat, calcium, fibrosis etc and thus helps the interventional cardiologist to choose or decide the appropriate hard-ware, like regular balloons, or balloons with blades or the use of drills like rotablator to drill through calcium, as required.

These technologies also help in perfect measurements of diameter and lengths of balloons and stents needed for that particular block or artery. This helps to provide the best results and good long term outcomes to the patients.

The video shows IVUS video run (2022) of a patient who developed a 99% block in the stent placed in 2008. He was treated with complex angioplasty and stenting with cutting balloon, supra-high pressure balloon and a new stent subsequently to provide the best results.

Dr Binoy John has been involved in performing both IVUS and OCT from 2011 and 2014 respectively and has very high experience with both and routinely uses them to provide the best results. He has authored chapters on this technology and given numerous national and international conferences.
