Salt Lake City, Utah — On October 29, 2025, Salt Lake City police responded to a 911 call reporting a man found injured in the roadway near 2100 South and 300 West. The caller disconnected twice after giving minimal details, stating only that the person was beginning to regain consciousness.

Upon arrival, officers spoke with multiple witnesses who confirmed an altercation had taken place. Officers then contacted 47-year-old Jose Hernandez, who was seated on a curb. He declined medical assistance and refused to answer questions regarding the incident.

The encounter escalated when Hernandez placed landscaping rocks inside his hooded sweatshirt. Officers instructed him to put the rocks down and remain seated, but he ignored commands, stood up, and walked away. He removed one rock, held it in his hand, and repeatedly refused to drop it despite clear orders.

Officers directed nearby bystanders to move away as Hernandez crossed 300 West and continued south. At the intersection of 2100 South and 300 West, Hernandez turned and threw the rock toward an officer. The officer responded by firing at least one round, striking Hernandez.

Officers immediately took Hernandez into custody and began providing lifesaving aid while requesting emergency medical services. He was transported to a hospital, where he later died. The involved officer sustained a non-life-threatening injury and was treated at a local hospital.
