It includes all your deposits,withdrawals, interest earned,service charges and penalties incurred in your your account.

Well this Bank Account Statement sample is from my HDFC Bank Account.
I have downloaded it a few days back from my HDFC internet Banking .
Ok, HDFC Bank, We understand your world.

Ok, in the left corner of the Statement lies the Customer's Address i.e your address and on the right corner lies the details of your Bank Branch from where the Bank Account Statement is issued.
Their Email Address, Branch Code and everything.

And see,this is the date.
I have blurred some portions intentionally due to security purposes.
That is why some portions are appearing blur.
But when you get your Bank Account Statement,it won't look blurred.
Well,OK, see, these are my transactions.
As I have not done any deposits within this time period, you can see that my Statement is showing only the withdrawals,all the IMPS that I have done,the AMB charges,the Dividend Yields that earned from my Demat Account.
This is all that you would find in a statement.
