On February 12-2009, A Colgan Air [Continential Connection] De Haviland Canada Dash 8 Q-400, registered N200WQ, operating as Continential Connection [Colgan Air] 3407 was on approach to Buffalo when the aircraft entered an unrecoverable aerodynamic stall and crashed into a house at "6038 Long Street, Clarence Center, NY", killing all onboard 😥
N200WQ was flying as 9L/CJC3407, operating from Newark Liberty International Airport [EWR] to Buffalo International [BUF].
The National Transportation Safety Board determines that the probable cause of this
accident was the captain's inappropriate response to the aerodynamic stall, others include;
(1) the flight crew's failure to monitor airspeed in relation to the rising position of the low-
speed cue,
(2) the flight crew's failure to adhere to sterile cockpit procedures,
(3) the captain's
failure to effectively manage the flight, and
(4) Colgan Air's inadequate procedures for airspeed
selection and management during approaches in icing conditions.
In this video, you can hear the F/O, Rebecca Lynn Shawn, acknowledging the handoff to Tower Frequency, at this moment, they were at 2300' feet, on an instrumental approach to Runway 23. 15-20 seconds later, at 22:16:27, the airspeed reaches 130kts, and at this same time, Captain Renslow asks for Flaps 15, to which the F/O replies "Uhh."
at 22:16:28, the Captain pulls the yoke toward him, as the speed reaches 120kts,
at 22:16:33, the airspeed reaches below 100kts, the flaps are at 10°, and the aircraft enters an aerodynamic stall. The captain is still pulling the yoke toward him.
at 22:16:36, the Dash 8 rolls into a 90° right bank angle, and continues rolling, the airspeed is 90kts, the First Officer also put the flaps to 0° [contributing factor]
at 22:16:40 the Captain manages to bring the aircraft to level wings, but this does not stay. the airspeed is 98kts.
at 22:16:45, the Dash 8 starts another right roll, this time, starting a nosedive, two seconds later, the Captain calls for gear up, the time is 22:16:47, the airspeed is 124kts.
The ATC radar returns after this approximate point became "XXX", meaning, "Unreliable".
at 22:16:49.50, the Airspeed suddenly starts falling back as the aircraft is 1400 feet above the ground, the highest airspeed in this nosedive was 138kts [approximately].
at 22:16:53, the FDR stops, the altituude is 740ft.
the CVR stops recording at 22:16:53.9.
The aircraft plows into a House in Clarence Center, killing all 45 passengers and 4 crew.
Factors suggested by the NTSB include;
-Fatigue
-Improper Training [will explain]
Captain Renslow had failed three check-rides before being hired by Colgan Air in 2005. Allegedly, he had a Tail-Stall test a month prior, which he failed.
Colgan Air defended Captain Renslow, stating, "Captain Marvin Renslow was fully capable and had the proper training to be in control of Flight 3407."
the Report also stated how fatigued Rebecca Lynn Shawn was, stating that she had a lack of sleep, and disorientation from her schedule that day. On the approach into Buffalo, the reports state that she clearly sounded fatigued.
