This Monster Caused a TPK Every Time I Ran It.

Every once in a while in D&D you stumble across a monster that just has the right synergy of unique abilities, that it totally catches the players off guard. This can then lead to a TPK, which is pretty much what has happened every time I’ve run the balhannoth. It’s not one of the most well known aberrations of Dungeons & Dragons, but in my book, it’s one of the greatest—in 3e, 4e, and 5e (each in its own way).
