✈️ Aviation has one of the lowest failure rates in the world - around 1 defect per million opportunities.

That’s 1 DPMO.

This precision has been achieved through relentless investment in safety systems, checklists, automation, and culture.

The result? In 2024, 318 fatalities across 40.6 million flights and 5 billion passengers (yet 2024 was rated the worst year in aviation history w.r.t to fatalities since 2018) - a tragic number, yes, nothing can measure the loss of a loved one. But also, an extraordinary engineering and procedural feat.

🩺 Now shift to healthcare—in the US, the world’s most advanced country.

In 2024, 371,000 people died due to medical misdiagnosis in the United States alone.

That’s over 1,000 preventable deaths a day - the equivalent of four Boeing 787s crashing daily.

Yet it barely makes headlines. Why?
Because we don't fear what’s familiar.
We fear what’s dramatic.

That’s recency bias, availability bias, and narrative neglect in action. This is not a post to point out at the US alone, but on Healthcare globally.

🔄 How can we shift this imbalance of attention?

Make the invisible visible
– Track and publish Defects Per Million Diagnoses (DPMD) just like DPMO in aviation.
– Normalize talking about errors without stigma.
Design for reliability, not heroism
– Build systems and protocols that don't rely on individual brilliance but reduce room for error, like aviation checklists. Train leaders to recognize bias in data interpretation
– Embed decision-making models that challenge what gets prioritized.
Tell better stories with data
– Data doesn’t move people. Narrative with insight does.
