Obtaining your client’s information is really important when you first start working with them, so you have an emergency contact for them, you know who their GP is and you know how to get hold of them. However, once you’ve started working with a client do you check in with them on a regular basis to see if any of their details have changed?

I think that the end of the year is a great time to do this check in and I’m going to show you how to create a quick Google Form to do just that.

FORM DETAILS:
We like to make sure our records are up to date a couple of times a year and wanted to reach out to see if any of your personal details had changed since you completed your Registration Paperwork.
Could you please kindly complete the below form so we can make sure we have all your up to date details.

Full Name
Contact Number
Address
Emergency Contact (name, relationship and contact number)
GP (name and telephone number)
Professionals Involved in your Care (Please list any even if we don't have a Release of Information for them)
