We're partnering with Santa Clara County, Abode Services, and UCSF researchers to support Project Welcome Home. Our goal is to provide 150-200 of the County’s highest-need chronically homeless individuals with permanent supportive housing.

We built a platform that integrates key social services data with built-in analysis tools to help case managers, program managers, and evaluators better identify the highest need homeless clients, locate and connect them to services, track their progress and monitor the overall success of the program.

Santa Clara County is fundamentally transforming how social services are delivered, and we're proud to support Project Welcome Home alongside our partners.
