The anterior inferior cerebellar artery (AICA) is one of the three main arteries that supply blood to the cerebellum. It originates from the basilar artery and courses posterolaterally, ventral to the pons in the prepontine cistern. The AICA then enters the cerebellopontine angle and continues along the anterior cerebellar surface, where it bifurcates into superior and inferior trunks at the pontomedullary junction.

The AICA can be divided into four segments: anterior pontine, lateral pontine, flocculopeduncular, and cortical. It supplies the middle cerebellar peduncle, lower lateral pons, anteroinferior surface of the cerebellum, and cranial nerves VII and VIII. The AICA exists in hemodynamic balance with the superior cerebellar artery (SCA) and posterior inferior cerebellar artery (PICA), with the sizes of these vessels being highly variable depending on the dominant arterial supply to the cerebellar cortex.

Anatomical variations of the AICA-subarcuate artery (SAA) complex are common. They can be classified into five grades based on the interaction between the petrosal bone and the AICA during embryonic development. These variations are important to consider during cerebellopontine angle surgery, as they can significantly impact the complexity of the surgical approach
