The 1975 Tân Sơn Nhứt C-5 accident was when a U.S Air Force Lockheed C-5 Galaxy carrying 314 people crashed near the Tân Sơn Nhứt airbase on April 4th, 1975.
Crash:
The plane was headed to Clark Airbase in the Philippines and while cruising over the South China Sea when the cargo door explosively opened cutting all the hydraulic lines, the pilots almost managed to make it back to the Tân Sơn Nhứt airbase using the thrust from the engines but during the final turn towards the runway the plane lost altitude, hit the ground, bounced back up, and slammed down just past a river causing the plane to break up killing 138 people on board leaving 176 survivors.
Investigation:
The original locks on the cargo door were used for scrapped and replaced with new ones that were improperly installed, maintenance workers were negligent in their inspections and 5 of the 7 locks didn't lock properly.
FedEx 14 was a scheduled cargo flight from Singapore to Newark, New Jersey with stopovers in Penang, Malaysia, Taipei, Taiwan, and Anchorage, Alaska carrying 3 passengers and 2 crew members on July 31st, 1997.
Crash:
While landing at Newark, the plane bounced back up into the air and upon the second touchdown the right landing gear failed which caused the plane to roll over on its back and catch fire, all 5 people escaped alive.
Investigation:
The Captain had pushed the nose down too far while landing which led to the plane bouncing, the over-stressed right landing gear strut broke which led to the collapse of the right landing gear and the puncturing of the right wing fuel tank.
Ansett New Zealand 703 was a commercial passenger flight from Auckland, New Zealand to Palmerston North, New Zealand on June 9th, 1995 carrying 21 people.
Crash:
The pilots had to manually lower the right landing gear with a hydraulic pump as it failed to deploy automatically, soon after the GPWS sounded 4 seconds prior to impact, the plane crashed into high terrain killing 4 people. Both pilots made it out alive.
Investigation:
The antennas on the front of the plane that sends information to the plane's GPWS was painted over, this severely reduced the sensitivity and accuracy of the GPWS letting the plane's descent go unnoticed by the pilots until it was too late.
Aeroperu 603 was a commercial passenger flight from Miami, Florida to Santiago, Chile with a stopover in Lima, Peru carrying 70 people on October 2nd, 1996.
Crash:
After take-off, the pilots received many erroneous warnings about the plane's altitude, airspeed, and location, the crew declared an emergency and tried to return to the airport but was unable to because of the false information, the ATC ordered a 707 to help guide the plane in but before it could do so, the 757's left wingtip struck water, the plane climbed but soon inverted and crashed into the ocean killing al on board.
Investigation:
When the plane was being cleaned prior to the flight, a mechanic accidentally left duct tape on the static ports of the aircraft, the static ports along with the pitot tubes measure the plane's altitude and airspeed so when these were covered with duct tape, they gave false readings allowing the plane to descend and climb without the pilots noticing.
ALM 980 was a commercial passenger flight from New York City, New York to St. Maarten on May 2nd, 1970 carrying 63 people.
Crash:
The weather around St. Maarten was worsening and the pilots made 3 landing attempts before deciding to divert to St. Croix, the plane ran out of fuel before making it to St. Croix forcing the crew to ditch in the Caribbean sea, the Public Adress System wasn't working on the plane so the Captain couldn't inform the passengers to brace for impact, in the ditching and eventual sinking of the plane, 40 people were rescued including the Captain and First Officer, 23 passengers and crew died.
Investigation:
The pilots failed to adequately monitor their fuel as they made several unsuccessful landing attempts at St. Maarten, the crew failed to divert earlier and they should've never taken off with a faulty Public Adress System.
