Why is it so important? In an era of EMR, big data, and machine learning, we're still using incomplete, static thresholds to define complex disease (for example, the "Sepsis Criteria"). The weaknesses of threshold -based definitions of disease include: Alarm Fatigue (if thresholds are highly sensitive); False Sense of Security (if thresholds are highly specific); Non Reproducible Clinical Trials (due to oversimplified input for statistical analysis; and Inability to Detect Transitions (e.g., super-infection, recovery). It is clear that, to save more lives, we need to re-evaluate the science in a way that embraces the complexity of pathophysiologic processes.
