Physical and psychological care of mother

1. General care
-Aseptic precautions
-Continuous emotional support, Encouragement and assurance
-Constant supervision

2. Careful examination
A complete and careful examination is done, including physical examination , pelvic examination & Laboratory tests are also performed.

3. Prevention of infection
Use aseptic techniques during vaginal examinations, and maintain proper hygiene of the woman to prevent infection.

4. Perineal preparation
Shave the perineal area, and perform proper toileting.

5. Rest
A woman in early normal labour may not be confined to bed. Advise the mother not to lie in the dorsal supine position.

6. Ambulation
If the membranes are intact, the patient is allowed to walk. Walking encourages the descent of the fetal head.

7. Bowel care
If the rectum feels loaded, a soap-and-water enema is given because a loaded bowel inhibits uterine contractions.

8. Bladder care
Encourage the patient to pass urine by herself to empty the bladder ,as a full bladder inhibits uterine contractions.
If she is unable to go to the washroom, bed pan may be provided. Catheterization may also be done.

9. Diet
-There is delayed emptying of the stomach during labour. Therefore, advise the woman not to take solid food.
-Fluids such as plain water, ice chips, or fruit juice may be given.
-In cases of prolonged labour, 5% dextrose may be started.

10. Relief of pain
Pethidine, an opioid drug, is an effective analgesic, given in a dose of 50–100 mg IM.
Metoclopramide 10 mg IM is commonly given to combat vomiting caused by pethidine.




13. Vaginal examination
Vaginal examinations should be kept to a minimum to avoid the risk of infection
(usually at least 4-hourly).
Findings during vaginal examination
Note the:
Dilatation of cervix
Position of head
Degree of flexion
Station of head
Colour of liquor
Degree of moulding of the head


14. Intake and output
Careful recording of the intake and output chart should be done every 4 hours.

15. Emotional support
The mother is supported emotionally
-Guided, and kept informed about observations and actions.
-Good IPR should be maintained so that the mother can verbalize her feelings during labour.
-Allow a family member or husband to stay with the mother for support.
