During a cardiac arrest the Advanced Cardiac Life Support (ACLS) team operates with clear role distribution to maximize the chances of survival. This in EMS typically is a mix of both advanced cardiac life support and basic life support providers that work as a team.

I share with you a useful command structure, led by a team leader, often an experienced paramedic, who makes critical decisions, and ensures communication among team members.

This leader assigns roles based on the members' skills and the requirements of the situation. Common roles include an airway manager, responsible for establishing and maintaining the patient's airway; a chest compression leader, who directs or performs high-quality chest compressions; a medications provider tasked with preparing and administering ACLS drugs; and a defibrillator/monitor operator, who manages the settings and delivery of defibrillation.

Regular training and simulations help maintain a department efficiency, ensuring that each member is prepared to perform their assigned roles under the high-pressure conditions of a real-life cardiac emergency.
