On August 22nd, 2024, the Bree Collaborative hosted a webinar on “Pre-diabetes Care Pathways to Connect Patients with Community Support”. During the event, speakers from UW Valley Medical and YMCA of Greater Seattle presented their collaborative and innovative strategies to enhance screening and referrals for patients with prediabetes to the Diabetes Prevention Program (DPP). They shared their successful approaches, including integrating screening into Ambulatory Care Pathways and targeted interventions to overcome social determinants of health barriers. Attendees gained insights into how they overcame challenges and achieved sustainability in clinicians’ engagement with prediabetes-centered referral opportunities, expanding their impact across various lifestyle change programs.
