On this day, the USS Forrestal—once the pride of the U.S. Navy’s carrier fleet—was transformed into a floating hellscape. At precisely 10:50 AM, as aircraft were being armed for a combat launch against North Vietnam, a Zuni rocket misfired from beneath an F-4 Phantom. In an instant, it slammed into an A-4 Skyhawk, rupturing a fuel tank and unleashing a torrent of JP-5 jet fuel across the flight deck.

What followed was an uncontrollable inferno. Bombs cooked off. Explosions rocked the ship. Sailors—many barely out of high school—ran into the flames to save their brothers. Others were trapped, surrounded by smoke and steel, waiting for help that never came. The fire raged for hours. When it was finally extinguished, 134 were dead. Over 160 wounded. Aircraft lost. Deck destroyed. Hearts broken.
