The Network of Care for Public Health Assessment and Wellness (www.networkofcare.org) is provided in partnership with the National Association of Counties (NACo) and the National Association of County and City Health Officials (NACCHO).

The Network of Care for Public Health won a prestigious national award in 2012 for Leading Health Indicators, presented by the U.S. Department of Health and Human Services and the Office of the National Coordinator for Health Information Technology, for helping solve the nation's high-priority health problems.
