This level really was being a nuisance in order to max it.
The problem with this level is, that by normal means, you can't access 100% items. Why?
Because how sequence-puzzle works:
A) You need to follow the sequence to know, what's the correct pillar out of the five, use switch to
pull one down with soulsphere, which also lowers blue key and then you can proceed.
B) If you choose wrong pillar, you get an armor bonus, which spawns 3 lost souls aswell and reset
the pillars.
Problem: 5 items spawn and you can only get one pillar down at a time.
When you get soulsphere, 4 armor bonuses won't disappear and you can't get those pillars down to pick them up.

Meaning, that normal 'perfect' run, would let you obtain max of 42% items.
(Roughly 14.285% per item. 7x14.285 = 100%)
In sequence-room: Soulsphere + 4 armor bonuses 5 items
Rest of the level: Soulsphere + Berserk-pack 2 items
[ Normal playthrough ]
Soulsphere (for blue key) + Soulsphere + berserk-pack = 3 items
Armor bonuses left on pillars = 4 items

I'm still unsure, how it actually breaks, but to get 100%:
a) Choose wrong pillar. Pick up armor bonus and run out of pillar-sector, before it hits the ground.
If done properly, that pillar stays down instead of going up again. (This doesn't always work)
b) Activate the randomization and the sequence again. Go stand on the edge of the pillar-sector,
that would normally be up. (Look at the floor here)
c) Get lucky with randomization and have soulsphere spawn on floor, next to you. (I believe you
can block the spawn, if you're too much inside the sector. Don't move too far, because then you
won't block the ceiling-part)
d) Pick up soulsphere and blue key. Kill lost souls.
e) Move to 5 switches and choose another wrong switch.
If everything went perfectly, that wrong pillar comes down with an armor bonus.
You can go pick it up to reset the pillars and get rid of extra items.
Thus, allowing you to actually get 100% items at the end of the level.

After all the trouble to solve this problem, I managed to do this on a previous video and then forgot to actually take the secret from the same room, because all of the repeated runs to break it.
Getting soulsphere at wrong point means having to reset the level.
