Treatment for a prostatic cyst depends entirely on whether the cyst is causing symptoms (like difficulty urinating or pain) and how large it is. In many cases, these cysts are found incidentally during scans for other issues and require no treatment at all.

1. Active Surveillance (Watchful Waiting)
If the cyst is small and you aren't experiencing any symptoms, doctors typically recommend regular monitoring. Since most prostatic cysts are benign (non-cancerous) and don't grow rapidly, your doctor may simply schedule periodic ultrasounds to ensure there are no changes.

2. Minimally Invasive Procedures
If the cyst is causing discomfort or obstructing urine flow, several "less invasive" options are available:

Aspiration: A doctor uses a needle (often guided by ultrasound) to drain the fluid from the cyst. While this provides immediate relief, the cyst may eventually refill with fluid.

Sclerotherapy: Following aspiration, a sclerosing agent (a chemical) may be injected into the empty cyst to help it "stick" together and prevent it from refilling.

Transurethral Deroofing/Marsupialization: This is the most common active treatment. A surgeon uses an endoscope (inserted through the urethra) to cut away the "roof" or wall of the cyst. This allows it to drain permanently into the urethra.

3. Surgical Options
For very large cysts or those located in hard-to-reach areas, more traditional surgery may be necessary:

Laparoscopic or Robotic Surgery: Small incisions are made in the abdomen, and specialized tools are used to remove the cyst. This is often preferred for "Müllerian duct cysts" which can be quite large.

Open Surgery: Rarely used today, but may be necessary for extremely complex or massive pelvic masses that cannot be managed with a scope.

When is treatment necessary?
You should consult a urologist if you experience any of the following, as they may indicate the cyst is interfering with your health:

Urinary issues: Frequent urination, weak stream, or feeling like your bladder isn't empty.

Pain: Chronic pelvic pain or pain during ejaculation.

Infections: Recurrent urinary tract infections (UTIs) or prostatitis.

Infertility: Some midline cysts can block the ejaculatory ducts, affecting sperm count.
