This video helps to resolve accident and incident investigation in a ISO 9001 and ISO 45001 company.
Hi friends welcome to QHSE Concepts, today we will discuss about an effective incident investigation to avoid any workplace incident.
There are lot of companies’ that carryout an incident investigation as a formality and they don’t do carry out a proper root causes analysis of the incident and the recommended corrective actions most of the time are not SMART (Specific, Measurable, achievable, realistic and time bond) enough to prevent the incidents from recurring.
If the incident is not being properly investigated, there is a high potential that a similar incidents could reoccur, and it can end up with a major loss/ damage to the company or an individual.
The purpose of accident/ incident investigation is to find the underlying causes of an injury, illness, property damage and near miss incidents and to take corrective actions as appropriate. Employees should understand that accident investigations are not intended to blame someone.
Causes of the incidents are, Unsafe Acts, Unsafe Conditions and Natural disaster, which can be due to human error, time limit, defective tools, horse play, inadequate procedures in place etc...
80-90 percent of all accidents / incidents are caused by unsafe acts and unsafe condition at the work place.
Accidents or injuries are the tip of the iceberg of hazards.
All incidents must be reported in a timely manner and properly investigated.
There are three causes to be considered while carrying out a Root Cause Analysis.
1. Direct Cause – Unplanned release of energy or hazardous materials
2. Indirect Cause – Unsafe acts and/or unsafe conditions
3. Root Cause – policy and decisions, personal factors, environmental factors
The “five whys” are the simplest of the root cause analysis methods. It is a question-asking method used to explore the cause/effect relationships underlying a particular problem.
Ultimately, the goal of applying the 5 Whys method is to determine a root cause of a defect or a problem.
Based on the Root causes analysis corrective actions are to be planned and assigned to prevent recurrence.
