Bhaiyya My Brother is about Satya (Allu Arjun) and Deepthi (Kajal Aggarwal) who are lovers living in Vishakhapatnam. A dreaded don, Veeru Bhai (Rahul Dev), lusts for Kajal and is infuriated by her relationship with Allu. Both of them decide to tie the knot and make a run for their lives, but Veeru has other intentions. They board a bus to their destination, only to be followed by Veeru's goondas, his brother and a corrupt cop. They manage to Kajal, and leave Allu almost Demise, stabbed multiple times and face partially disfigured. However, Dr. Shailaja (Jayasudha) saves Allu by giving him a new face and skin which is that of Ramcharan. He escapes from the hospital to avenge his lover’s . How he manages to avenge her and the real story behind the face of Ramcharan forms the rest of the story.
