Small Intestinal Bacterial Overgrowth (SIBO) occurs when there is an increase in the bacteria population in the small intestines.
This is an important discussion to understand because the small intestine is where the food goes once it’s done being processed into a liquid acid by the stomach. The small intestines, known as the “kitchen area” is where the food is now absorbing into the bloodstream, thus it needs to be fairly sterile and free of pathogenic bacteria. It contains only about 10,000 bacteria per ml of fluid. In contrast, the large intestines, known as the “garbage area” is where there are lots more bacteria to work breaking down the food into your stool and it therefore contains about 1 trillion bacteria per ml of fluid.
Another important fact is that the small intestines is 20 feet long, yet the food only stays in there for two hours. The large intestines are 5 feet long, yet the food stays in there for 2 days as the bacteria work on it. So you can see how we were made to keep the small intestines free of bacteria.
The video discusses four main reasons that infection might grow in the small intestines.
1) Lack of friendly bacteria in the gut can encourage the growth of bacteria in the intestines since preventing the growth of pathogenic organisms in the gut is one of their main functions. These important bowel flora can be depleted through the use of antibiotics, birth control pills, acid reflux medicines, steroids and vaccines. Therefore, if you are taking any of these medications you should know the best types of probiotics and/or yogurt starter cultures (to make your own home-made yogurt, lassi, buttermilk and takra) to prevent any overgrowth of unwanted organisms which might come as they kill off your friendly bacteria.
2) The use of acid reflux medicines: Once you swallow the food it goes directly into the stomach, where the environment is highly acidic due to the presence of hydrochloric acid which churns your food into a liquid acid. This first step in the digestion process is very important, not only to correctly break the food down into a liquid for best absorption into the bloodstream, but the acids in the stomach kill any infection which might be entering the body from the outside world, which is teeming with bacteria.
Acid reflux medications take away the stomach acid, which could increase the risk of you developing SIBO since potential infections will not be killed in the stomach if the acids are missing, and they could possibly create small bacterial intestinal overgrowth as they continue on into the small intestines.
3) The third reason for SIBO is lack of movement through the gut. The gall bladder secretes bile which creates two types of movement through the intestines. The first type of movement, known as peristalsis, occurs when you eat food. These are gentle contractions which happen fairly soon after swallowing the food. A second type of movement known as the Migrating Motor Complex (MMC) also occurs due to the flow of bile out of the gall bladder. But this type of movement happens a few hours after eating to push the food through. These electoral waves, known as the “Housekeeping Waves” are designed to keep the food moving through the small intestines so no infection can set in.
However, many times in a lifetime the bile might not flow if it develops a thick sludge and these two types of movements might not occur effectively enough to push the food through the gut quickly, resulting in SIBO.
4) An open ileo-cecal valve (the valve separating the contents of the large and small intestines) could allow food from the large intestine (the garbage area) to regurgitate back into the small intestines (the kitchen area) causing infection to grow in the small intestines. The open ileo-cecal valve occurs from problems higher up in the digestive processes, so all attempts must be made to address any digestive issues occurring at the level of the pancreas, liver, stomach, gall bladder or gut.
