Why don't cyclists use cycle lanes?

It's very common to see cyclists riding in the middle of the road when there is a perfectly good bike lane on offer to them – but why, and is it even legal? Here's six reasons why people riding bicycles in the UK might find it safer or preferable to ride in the road, rather than the bike lane – from bad design or poor maintenance, to safety and speed.
