That is a very important question. I did say that IVF is the most successful treatment that is available today. But what is important to understand is that IVF is expensive and it does have some risks associated with it. So it must be done only when it is really necessary. In this regard, the IVF is advised only if the woman has blocked tubes, which cannot be corrected surgically. Second when someone has very low sperm counts and which are not adequate for IUI, then IVF is advised. In cases such as unexplained infertility, mild male factors or minimal or mild endometriosis, then IVF is done if there has been more than 2 years of trying and if 4 IUI’s have not worked, or a situation where the women needs a donor egg or a surrogacy, then an IVF has to be done. In cases of Polycystic Ovarian Syndrome if it has been a year where ovulation has been happening continuously and IUI’s are nor working then IVF is usually done. In cases of severe endometriosis, we do advise IVF and if someone has no sperms in the semen and if surgical retrieval of sperms has to be done, the IVF has to be done in such couples.
