The #OurMN campaign takes a look at the Minnesota Department of Human Service's Return to Community initiative for Minnesota's aging population.

Return to Community gives seniors a chance to transition out of nursing homes and back into their own homes and communities.

Started in 2010, the initiative saves taxpayers money by helping seniors get services in their own homes, rather than through a nursing home.

The program is a partnership between Minnesota Board on Aging, Minnesota Department of Human Services, and nursing homes across the state.

Community Living Specialists visit eligible people in nursing homes to discuss whether they are interested in getting help to move back home.

The Senior LinkAge Line® service is provided by six Area Agencies on Aging that cover all 87 counties of Minnesota and helps connect you to local services. The Senior LinkAge Line® is answered from 8 a.m. to 4:30 p.m. weekdays.
