The input mask (UserForm) is created in the VBA editor. The VBA editor in Excel can be opened in two common ways: by pressing Alt+F11 or from the ribbon by right-clicking on the sheet tab and selecting "View Code".

In the VBA editor, select "UserForm" from the "Insert" menu, UserForm1 will be created automatically.

Go to the worksheet. Left-click on the "Design Mode" symbol to activate it. Double-click on the button. In the window that opens (Sub), enter the code found in the description of this video.
