Case Note:
Setting: Hospital Outpatient Clinic
You see the son/daughter and primary carer of a 75-year-old woman. His/her mother has dementia and has recently had a cholecystectomy (surgery to remove the gall bladder) and is now being discharged. You are discussing the required aftercare. His/her mother is not present.

Task:
•⁠ Confirm reason for your visit (information about mother's aftercare post- discharge). Find out about mother's living arrangements.
•⁠ ⁠Describe benefits of living with family (e.g., practical/emotional support, social interaction, etc.). Explore son/daughter's view of mother's recovery so far (physical strength, energy, mood, etc.).
•⁠ ⁠Emphasize necessity of extended stays for some patients (e.g.. age, slower recovery, etc.). Give details of mother's progress (e.g., no infection, improvement in appetite, minor discomfort, etc.).
•⁠ ⁠Explain mother's wound care (e.g., cleaning daily, keeping covered, soluble stitches: disappear in 2-3 weeks, etc.). Outline medication (e.g., oral antibiotics for five days, pain relief as required, laxative twice daily unless stool is loose, etc.). Describe restrictions (e.g.. no lifting, gentle exercise, small/frequent meals, etc.).
•⁠ ⁠Reassure son/daughter about post-discharge information (e.g., in discharge pack, etc.). Outline further support available (e.g., community nurse, mother's doctor, etc.).
