With HRT, the most common approach is low dose - static. This is for several reasons:
1. Just the tiniest whiff of estrogen is usually enough to treat hot flashes
2. Low dose = no bleeding = much easier to prescribe, especially via telehealth
3. The Women's Health Initiative convinced so many people that hormones were dangerous that "the lowest dose for the shortest time" became the mantra.

None of these reasons are to optimize women's healthy longevity because, not surprisingly, when it comes to estrogen, dose matters. At physiologic levels, estrogen creates growth factors that rejuvenate the brain, bones, and cardiovascular system. These are the same growth factors that cause the uterine lining to growth. This is just how it works. To rejuvenate the brain, you have to grow the uterine lining.

So you must pair physiologic estrogen with cyclic progesterone and a monthly, period-like bleed.
