The Reticulocyte Count Test is an important haematology investigation used to measure the number of immature red blood cells (reticulocytes) in peripheral blood. It reflects bone marrow activity and RBC production rate, making it essential in the diagnosis and monitoring of anaemia, especially hemolytic anaemia and in response to treatment.

🔬 Principle

Reticulocytes contain residual RNA that can be visualized using supravital stains (e.g., New Methylene Blue or Brilliant Cresyl Blue). When stained, the RNA precipitates into a dark blue reticular (mesh-like) network inside the cells. These stained cells are counted microscopically to determine the percentage of reticulocytes among total RBCs.

🧪 Procedure (Supravital Staining Method)

1. Mix a few drops of New Methylene Blue stain with an equal volume of EDTA blood.


2. Incubate the mixture for 15–20 minutes at room temperature.


3. Prepare a thin smear on a clean glass slide and allow it to air dry.


4. Examine under oil immersion (100x).


5. Count 1000 RBCs, noting the number of reticulocytes seen.

↑ High Hemolytic anaemia, acute blood loss, response to iron/B12 therapy
↓ Low Bone marrow failure, aplastic anaemia, chronic kidney disease, chemotherapy


This test is trending as a key indicator of bone marrow recovery, especially after anaemia treatment, chemotherapy, and bone marrow transplant.
