1. Passport
2. Certificate of Eligibility (COE) (original and photocopy)
3. One passport-sized photograph
4. Completed Visa Application Form to Enter Japan
5. Written Pledge (2 color copies)
6. COE Reissue Letter (If required by the consulate)
